Instructions for sending biopsy slides for consultation

Request that slides and report (must have pathology report) be sent by either Airborne, Federal Express (these two are the preferable carriers), or UPS (do not send by regular mail) to:

The package should be sent to the attention of the pathologist that you would like to review your slides. The slides may be picked up from the outside institution and brought directly to the consult office.

 The following information is needed before your consultation can be processed:

  1. Patient’s full name, date of birth, social security number, address, telephone and fax number.
  2. Insurance information.
  3. The full name, address, telephone and fax number of the physician (s) to receive a report.
  4. Indicate if you would like a report faxed or mailed to your home. Reports may not be e-mailed.
  5. Instructions on returning slides (i.e., original institution, forwarded to another institution). Must have the complete address (no P.O. numbers), telephone number (name is optional unless there is a particular person the slides should be sent to). The slides may also be picked up in the consult office.
  6. Letter indicating any pertinent information that pertains to your history.

The consultation fee is $200 and may be covered by insurance. If not, the fee may be paid by check payable to: The Johns Hopkins University. We accept major credit cards and need the account number, company name and expiration date. This information may be given at the time the specimen is received in order to maintain confidentiality (indicate if you choose to pay by credit card). There may be an additional charge if additional studies are performed.

The report is usually ready within two days unless additional studies are needed. A report will be automatically faxed to the physician as soon as the case is released. The slides are held for approximately 10-30 days (60 days if being treated at Johns Hopkins) and then returned. If the slides are needed before that time, the outside facility (i.e., hospital, treatment center, physician) may fax a request to our office at the above number (include name, address, telephone number).
